What your report is actually telling you

"Grade 1 fatty liver" means the ultrasound can see a mild amount of fat inside your liver cells. Grade 2 means moderate fat. Grade 3 means a lot.

The grade describes fat. It tells you very little about damage.

That's why doctors usually stay calm about grade 1. Fat on its own is the early stage that responds well to food changes. What they watch for is inflammation and scarring (doctors call scarring fibrosis), which a regular ultrasound can't measure. A FibroScan or a blood-based score can.
