Low Testosterone Symptoms in Men: 12 Signs & When to Get Tested

Low Testosterone Symptoms in Men: 12 Signs & When to Get Tested

The low testosterone symptoms that tell you the most show up in your sex life. A drop in desire that lasts for months is a strong clue, especially when your morning erections have mostly disappeared. Tiredness and low mood can come from low T as well, but they usually have other causes. The only way to know for sure is a morning blood test below roughly 300 ng/dL, repeated on a second day.

Most symptom lists online give you 15 or more signs and treat every one of them the same. So you read one, tick half the boxes and start worrying.

Some signs carry a lot of weight. Others barely mean anything on their own.

What low testosterone actually means

Testosterone is a hormone made mostly in the testicles. Women make it too, in much smaller amounts, in the ovaries and adrenal glands. It looks after your sex drive and muscle. It also keeps your bones strong.

Doctors call it low T (or hypogonadism) when two things happen together. Your blood level sits below about 300 ng/dL. And you have symptoms. Plenty of men have a low number and feel completely fine, so a lab result on its own doesn't settle it.

Belly fat and type 2 diabetes raise the odds a lot.

Low testosterone symptoms ranked by how much they tell you

The European Male Ageing Study followed over 3,000 men aged 40 to 79. When researchers matched symptoms against blood levels, the sexual symptoms stood out. Most of the others overlapped with ordinary tiredness and stress.

Symptom

How strong a clue

Worth knowing

Fewer morning erections

Strong

One of the most specific signs in the research

Low sex drive for months

Strong

A dip during one rough month doesn't count

Trouble getting or keeping an erection

Strong

Also linked to blood sugar and heart health, so get it checked anyway

Smaller or softer testicles

Moderate

See a doctor about this whatever the cause

Breast tissue growth

Moderate

Some medicines cause it too

Losing muscle even though you train

Moderate

Check your protein intake and sleep first

Constant tiredness

Weak

Poor sleep and anaemia are far more common causes

Low mood or irritability

Weak

Stress and depression feel almost identical

Brain fog

Low B12 is a frequent culprit

When the sexual signs turn up together, that's the pattern worth testing for. One sign on its own is rarely enough.

Common problems that feel exactly like low T

You're exhausted and your motivation has gone quiet, so it must be hormones. Often it's one of these instead.

  • Short or broken sleep. A well known University of Chicago study found that a week of 5-hour nights lowered daytime testosterone by 10 to 15% in healthy young men. Fixing sleep is the cheapest thing you can do. A magnesium glycinate tablet before bed helps some people wind down.
  • Low vitamin D. It's very common in India, even in sunny cities, because most of us work indoors.
  • Low B12. Vegetarians are especially prone to it. It causes fatigue and fogginess that look a lot like low T.
  • An underactive thyroid. It slows your energy right down.
  • Long-term stress. High cortisol pulls testosterone down over time. Herbs like ashwagandha have been studied for stress. A good routine matters even more.
  • Rising blood sugar. Insulin resistance and low testosterone tend to travel together. Our healthy food list is a sensible place to start with diet.

Ask for vitamin D, B12, TSH, HbA1c and a CBC in the same blood draw as your testosterone. One needle, far more answers.

Try the 14-day check before you book a test

Writing your low testosterone symptoms down for 2 weeks shows you if it's a real pattern.

Each morning, note these 5 things:

  • Did you wake with an erection? Yes or no.
  • How many hours did you sleep?
  • How was your desire yesterday, on a scale of 1 to 5?
  • How was your energy at 3 pm, on a scale of 1 to 5?
  • Did you drink alcohol last night?

After 14 days, look for the pattern. Missing morning erections on most days and steady low desire mean a blood test is a good idea. Symptoms that rise and fall with your sleep or your drinking point to a lifestyle fix first.

Take the log to your appointment. Doctors can do a lot more with real data.

What causes low testosterone

Testosterone drops slowly with age, roughly 1% a year from your 30s or 40s. That alone rarely causes symptoms. The bigger drivers are extra body fat and type 2 diabetes. Sleep apnoea and long-term opioid use play a part for some men too.

In younger men

Low T in your 20s usually has a clear reason. Past use of anabolic steroids or SARMs is a big one, because they can shut down your body's own production for months. Crash dieting and serious sleep loss also play a part.

Low testosterone symptoms in women

Women can have low testosterone too, especially around menopause or after the ovaries are removed. The signs are quieter: low desire and low energy that don't lift. Testing for women is less standard, so a gynaecologist or endocrinologist is the right person to ask. Some women look at maca for desire. We've gone through the research in maca root for women.

How to get tested in India

Proper timing decides how much you can trust the result. Most big labs offer a total testosterone test, usually for somewhere between ₹500 and ₹1,000.

  • Book it for between 7 and 10 am, when levels peak.
  • Go fasted if you can.
  • Skip a heavy workout or a sleepless night the day before.
  • Repeat it on another day before anyone calls it low.
  • See a urologist or an endocrinologist to make sense of the numbers.

Reading your report

Marker

What it tells you

Total testosterone

Your overall level and the first number doctors check

Free testosterone

The part your body can actually use, which is useful when total T is borderline

SHBG

A protein that binds testosterone, so high SHBG can hide a low free T

LH and FSH

Points to where the problem starts, which shapes the treatment

Some labs report in nmol/L. Multiply by 28.84 to get ng/dL. A result between 250 and 350 ng/dL is a grey zone, which is exactly why doctors repeat the test.

What genuinely helps

Losing excess body fat has the strongest effect of anything you can do yourself. Sleep comes next. Strength training and cutting back on alcohol both help too.

Supplements come after those. Here's what the research supports.

Ingredient

What studies show

What it won't do

Ashwagandha

Modest rises in small trials, mostly in stressed or overweight men

Treat diagnosed hypogonadism

Tongkat ali

Small trials show benefits, mainly when levels start low or borderline

Replace medical treatment

Fenugreek

Mixed results, with small rises in some trials

Work for everyone

Maca

May support desire

Change testosterone levels in studies so far

Zinc and vitamin D

Help when you're actually deficient

Do much when your levels are already normal

Our Testo Tablets are built around tongkat ali and fenugreek alongside 4 other traditional botanicals, with no synthetic hormones. They support your body's own processes within a normal range. They won't fix a medical deficiency. You can trace where every ingredient comes from on our ingredients page and see our lab certificates.

Testosterone replacement therapy

For confirmed low T, a doctor may prescribe testosterone replacement therapy (TRT). The catch is that it can switch off sperm production, so tell your doctor if you're planning a family. Never start it without a prescription.

When to see a doctor

Low testosterone symptoms that stick around after you've sorted out your sleep and stress deserve a proper check. Book an appointment if any of these apply:

  • The sexual signs have lasted more than 3 months
  • Your testicles have changed in size or feel
  • You've noticed breast tissue growth
  • You've been trying to conceive for a year without success
  • A properly timed test came back low
  • FAQs

What does low testosterone do to a man?
It mainly lowers sex drive and causes fewer morning erections. Over time it can reduce muscle and bone strength. Many men also feel more tired.

How can I naturally increase my testosterone?
Losing excess body fat helps most. Sleeping 7 to 8 hours and lifting weights both support healthy levels. Cutting back on alcohol helps as well.

Can stress cause low testosterone symptoms?
Yes. Long-term stress raises cortisol, which can lower testosterone. Stress also causes tiredness and low desire on its own, so check both before blaming hormones.

What is a normal testosterone level?
Most labs consider 300 to 1,000 ng/dL normal for adult men. Ranges vary slightly between labs, so always compare your result with your report's reference range.

Does TRT affect fertility?
Yes. Testosterone therapy can stop your body making sperm, sometimes for months. Men planning children should discuss fertility-safe options with a doctor before starting any treatment.

Fix your sleep first, then let a repeated morning blood test make the call.
